Our Case Managers hold a special role at our organization. Case Managers are tasked with cultivating relationships with clients to learn about their stories and advocate for their success. They work closely with clients to develop plans specific to each person’s individual needs and facilitate services offered at JCCM that will support them in achieving their goals.
Some areas that Case Managers assist with include:
· Obtaining birth certificates
· Obtaining state identification
· Housing applications
· Social security
· Referrals for health care
· Coordinating treatment for substance abuse disorders
Our Case Managers are mentors for our clients, but they also act as a caring beacon of support. They genuinely care for the well-being of our clients and revel in the work they do for our community.
Case management services can be obtained by speaking to an Intake worker about setting up an appointment. Our Intake department hours are Monday-Friday from 9:00 a.m. to 11:45 a.m
